• Website Update Submission Form: Online:  Use this form to request updates to the website.

PTA Treasurer -Related Forms (for PTA expenses or deposits only; not for grade/classes)

• Expense Reimbursement Form:  Online (strongly preferred):  Please note you can upload copies of receipts.

• Cashier/Cashbox Form:   Use this form when if you are chairing an event that requires a cash box (i.e. ticket sales or food sales).

•  Use the 501C3 letter above when you need to purchase classroom supplies or other PTA-related or funded items to receive tax exempt status and avoid paying sales tax.
